Proactive Health Care: Our Free Support Programmes for
Medical Card & GP Visit Card Holders

If you are already living with a long-term health condition, this programme is designed to give you extra care, regular monitoring, and peace of mind. It focuses on four specific areas: Asthma COPD (Chronic Obstructive Pulmonary Disease) Type 2 Diabetes  Cardiovascular Conditions (such as, history of heart attack, stroke, or an irregular heartbeat like atrial fibrillation) What you are entitled to: If you are aged 18 or over and have any of these conditions, you are entitled to two free structured reviews every year. Each review is broken into two relaxed steps: A visit with our Practice Nurse: They will check your vitals, do any necessary blood tests, carry out lifestyle checks, and talk through how you are feeling. A review from your GP: Your doctor will review your results, look over your medications, and amend if needed.

You do not need a diagnosis to get extra health support. The Prevention Programme is all about catching things early and working together to reduce your risk of developing heart conditions or diabetes down the road. This programme is specifically for: Adults aged 45 and over who may be at a higher risk for cardiovascular disease or diabetes. Adults aged 18 and over with a diagnosis of high blood pressure (hypertension). Women who have experienced gestational diabetes or pre-eclampsia during pregnancy since January 2023.     What you are entitled to: You receive one free comprehensive review every 12 months. We will look at your cholesterol, blood sugar, and blood pressure, and offer friendly, practical guidance on lifestyle, nutrition, or treatments to keep your health on the right track.

Think of this as a quick, helpful health screening that happens naturally. If you are aged 45 or over and visiting us for something else (like a prescription renewal or an unrelated check-up), your GP or nurse might suggest a quick OCF assessment if you aren't already on one of the programmes above. What it involves: It is a simple, stress-free check of things like your blood pressure, pulse, and BMI, along with a quick chat about your lifestyle. If the results show you might benefit from closer monitoring, we can register you for the Prevention Programme or the CDM Treatment Programme right away.
